Slushies are a summer and vacation staple for many children, known for their icy texture, sweet taste, and the telltale blue tongue they often leave behind. However glycerin, the additive that helps give these drinks their characteristic semi-frozen consistency, can cause health problems when consumed in large amounts.
According to a new assessment by the European Food Safety Authority (EFSA), even a single large slushie may be enough to exceed the safe dose for a single serving. The German Federal Institute for Risk Assessment (BfR) is now warning against consuming them. Clinicians should also consider whether a slushie consumed shortly beforehand may be the cause if children present with sudden nausea, vomiting, or drowsiness.
Slush ice is known by different names around the world:

- Italy: Granita
- France: Granité
- Spain: Granizado

A Food Additive Under the Microscope
Glycerin (E 422) is a colorless liquid with a slightly sweet taste. The molecule occurs naturally in vegetable and animal fats. In the food industry, glycerin has been used for decades as a humectant, solvent, and stabilizer. Therefore, it does not pose a classic toxicologic hazard.
The critical issue lies rather in the dose. Unlike many well-known food additives, glycerin has a pronounced pharmacologic effect. Medicine has used this property for many years. High-dose glycerin is used, for example, to reduce elevated intracranial pressure or elevated intraocular pressure.
This works because glycerin has an osmotic effect. After ingestion, it temporarily increases the concentration of dissolved particles in the blood, drawing water from tissues into the bloodstream. It is precisely this mechanism that is used therapeutically to draw fluid away from the brain or the eye, thereby reducing pressure.
Refreshment With Consequences
However, if the same effect is unintentionally triggered after drinking a slush ice beverage with a high glycerin content, side effects can occur. These include headaches, nausea, vomiting, dizziness, drowsiness, and diarrhea.
In isolated cases, children have even experienced temporary disturbances in consciousness. These symptoms essentially correspond to the known side effects of medical glycerin therapy and are the reason why experts today evaluate not only chronic intake but also, explicitly, high single doses.
Why Slush Ice Is Affected
Some foods contain glycerin but only in small amounts. With slush ice, the situation is different. The additive is used to ensure that the beverage remains soft and palatable despite temperatures below freezing. Without glycerin, the contents would freeze into a solid block of ice.
As a result, individual beverages can contain comparatively high concentrations. At the same time, slushies are often consumed within minutes. This combination of high concentration and rapid intake makes them problematic from a toxicologic perspective.
Children Reach Critical Doses Particularly Quickly
The risk depends directly on body weight. While adults can usually tolerate larger amounts, children can reach a critical dose after consuming much smaller quantities.
The BfR had already shown in 2025, based on measurement data, that children could ingest amounts equivalent to a therapeutically effective dose from as little as 200 mL of slush ice.
The EFSA has now evaluated these findings across Europe and, for the first time, established an acute reference dose of 125 mg of glycerin per kg of body weight for a single intake of glycerin-containing beverages. According to the agency’s calculations, this single dose would be exceeded by typical consumption of 250 mL in children and 500 mL in adolescents and adults.
Why Hardly Anyone Has Paid Attention to the Problem So Far
During the 2017 reevaluation of glycerin, the EFSA found no reason to impose a quantitative limit. At that time, the analytical data from beverages was significantly lower.
Only after measurement programs by German food safety authorities and numerous case reports of children developing sudden neurologic symptoms after consuming slush ice was a new risk assessment conducted. The BfR had already warned in 2025 of potential health consequences. The new EFSA opinion confirms this assessment and explicitly recommends that the European Commission establish maximum levels for glycerin in beverages in the future.
Consider Slush Ice as a Possible Cause of Acute Symptoms
For physicians, the risk assessment has one major implication: In children with sudden-onset headaches, nausea, vomiting, drowsiness, or dizziness, the medical history should specifically include questions about slush ice beverage consumption. The symptoms can occur shortly after consumption and are attributable to the pharmacologic effects of high amounts of glycerin, not to classic food poisoning.
Especially in younger children, even a relatively small amount of the drink can be enough to reach critical doses. The EFSA therefore recommends limiting glycerin intake from a single beverage and calls for legally mandated maximum levels in beverages.
