Health authorities across Europe are sounding the alarm over an unexpected resurgence of diphtheria — including a sharp increase in cutaneous infections — after decades of near elimination. The rise in cases has prompted renewed calls for adult vaccination and greater clinical vigilance among healthcare providers.
Diphtheria is an acute bacterial infection most often caused by Corynebacterium diphtheriae. Its severity stems from a potent toxin capable of damaging the heart, kidneys, and nervous system. The classic form, respiratory diphtheria, typically presents as febrile pharyngitis with pseudomembranes, while the cutaneous form produces greyish-white membranes on preexisting wounds or ulcers. Detection and Management Protocols
The management of suspected diphtheria requires strict compliance with public health protocols:
- Sample collection: Obtain pharyngeal or skin swabs for culture and polymerase chain reaction (PCR) testing for the tox gene. If necessary, promptly forward isolates to the National Reference Center at Institut Pasteur for confirmation.
- Isolation and infection control: Implement patient isolation and protective measures for healthcare staff until two negative samples — taken at least 24 hours apart — or 24 hours after treatment cessation.
