The word is out in the medical community that quieter hospital environments offer significant benefits to inpatients.
Most commonly, a quiet room leads to reduced anxiety and stress, with constant noise heightening discomfort and emotional distress, and a quieter setting also promotes better sleep quality, which is essential for healing, immune function, and cognitive clarity, said Bhavin Patel, DO, a hospitalist at UMass Memorial Health and an assistant professor at UMass Chan Medical School, both in Worcester, Massachusetts.
Not only are hospitalists on the forefront of inpatient care but also their role crosses over to implementation of better standards. Quieter hospital settings continue to be a goal of medical centers across the US. Here’s how the hospitalist specialty can keep this momentum charging ahead.
Why Quieter Environments Help Patients Clinically
Studies show that when hospitalized patients are more at ease, their physical and mental health can benefit. One component of a patient’s overall well-being is the level of rest and sleep they can achieve when they are in a hospital. Patients not only are able to get rest naturally but also can rely less on clinical interventions.

“Improved sleep environments can additionally reduce the need for sedative medications, anxiolytics, or antipsychotics, particularly in older adults who are vulnerable to delirium,” said Patel.
Another benefit of quieter settings in the medical arena is less interference.
“Less noise also means fewer distractions for both patients and staff, allowing for clearer communication and safer care delivery,” he added.
Additionally, lowering the volume can also result in shorter stays, Patel said.
The metrics also have demonstrated that less stress can contribute to improved patient outcomes. Hospitalists are well positioned to advocate for quieter environments because of their continuous presence and oversight of inpatient care, he said. Reduced interruptions during sleep can positively influence a patient’s mood and enhance overall sleep quality.
“Also, hospitalists can help minimize unnecessary noise by reducing continuous pulse oximetry or telemetry when not medically indicated, thereby limiting alarm-related disturbances at night. Retiming medications, including antibiotics, so they do not run overnight and trigger beeping is another effective strategy,” Patel said.
How Quiet Environments Intersect With Hospitalist Medicine
Exposure to noise, disruption, and excessive light may adversely affect patients, contributing to increased anxiety, persistent sleep disturbances, and diminished mental and physical well-being.

“I think anyone who has been admitted to the hospital has likely had the experience of being constantly interrupted at all hours of the day,” said Denise Xu, MD, an assistant professor of clinical neurology at the Perelman School of Medicine at the University of Pennsylvania in Philadelphia. “In speaking with some of our neurology patients recently about factors that affect their sleep in the hospital, what came up frequently aside from environmental noise and light was really the frequency with which people come in and out of their rooms, all day and night.”
To that point, although there are so many staff in a hospital setting who are crucial to a hospital functioning, from the clinical staff — physicians, nurses, nursing assistants, medical students, and technicians — to nutrition services to environmental services, and they all interact with patients in some way, patients are being affected because staff schedules don’t always match a patient’s schedule, she said.
“It really ends up being a constant flow of interruptions and a flow that is coming at unexpected times. That can be pretty disorienting for patients and, in a situation where one is ill and vulnerable and potentially unsure of what the future holds, lead to figurative and literal unrest,” added Xu.
Xu said her team has been finding ways to target the major contributors to poor sleep in the hospital, and what they’ve found through testing interventions and talking to patients, their caregivers, and hospital staff is that being thoughtful about the risks and benefits of allowing an uninterrupted, quiet period of time at night, allowing for patients to sleep, “certainly leads to more patient satisfaction.”
She said studies have also shown that with sleep-promoting interventions, patients report improved emotional and mental health but also staff satisfaction.
Hospitals have tried numerous ways to create quieter environments. For instance, a new building at the Hospital of the University of Pennsylvania — The Clifton Center for Medical Breakthroughs — incorporated a number of these innovations, including windows that can be frosted/defrosted from outside the room to allow nurses to check in on patients and make sure they’re safe without physically entering the room, specially designed doors, single-occupancy rooms, and clustered nursing stations to reduce noise from staff conversations, she reported. “But of course not every hospital may be able to invest in these kinds of structural changes,” said Xu.
What Is the Role of Hospitalists?
For hospitalists especially, because they interact with patients, advocating for patients can also streamline better care. Hospitalists are not only medical professionals with whom patients can share their concerns but also sympathetic and trusted providers. Making a patient feel comfortable because there’s a sympathetic ear means a lot in patient care.
“Hospitalists can build trust by being present, attentive, and transparent in their communication,” said Patel. “Simple actions such as sitting in a chair at the bedside rather than standing over the patients can make interactions feel less rushed and more personal.”
Moreover, listening without interruption, validating patients’ feelings, and acknowledging fears or uncertainties help patients feel respected and heard. “Taking time to explain diagnoses and treatment plans in clear, layperson’s terms is essential for understanding and reassurance,” he also said.
What Kinds of Messaging Can a Hospitalist Use?
Messaging should emphasize partnership and support, said Patel, such as:
“Your concerns matter to us.”
“Please tell us if something is bothering you.”
“We want to make this hospital stay as comfortable and safe as possible.”
Demonstrating empathy, maintaining clear communication, and ensuring consistency are essential for building trust.
Even more, consider an advisory board to take a more active role because many hospitals offer patient advocacy services along with patient experience and advisory programs. At UMass Memorial Medical Center, for instance, there’s an Office of Patient Advocacy staffed by licensed clinical social workers and registered nurses and a dedicated Patient Experience Department and a Patient and Family Advisory Council program. “Together, these teams represent the patient and family perspective and help elevate and address concerns related to comfort, communication, and care delivery,” said Patel.
Concrete Steps for Quieter Settings
Patel said hospitals can reduce noise through these practical measures:
- Establishing and enforcing quiet hours at night
- Dimming or turning off hallway lights overnight
- Limiting conversations in hallways and near patient rooms
- Reducing unnecessary telemetry, pulse oximetry, and alarm triggers
- Encouraging “six-inch voices” in clinical areas
- Avoiding routine medication administration between 11:00 PM and 6:00 AM when clinically appropriate
What Are the Real-World Impacts?
From a clinical perspective, a quieter hospital environment supports recovery in several important and very practical ways. Among the most important benefits is improved sleep quality.

“Hospitalized patients frequently experience fragmented sleep due to alarms, hallway noise, overhead paging, and frequent overnight interruptions,” said Kenneth E. Remy, MD, MHSc, MSCI, the Ellery Sedgwick, Jr Chair and Distinguished Scientist in Cardiovascular Research and associate professor of medicine, pediatrics, biochemistry, and pathology, as well as center director of The Blood, Heart, Lung, and Immunology Research Center at Case Western Reserve University in Cleveland. “Poor sleep can worsen pain perception, impair mood, and reduce a patient’s ability to engage in care during the day.”
So when noise is reduced, patients often report deeper and more restorative sleep, which supports healing and participation in physical therapy, pulmonary care, and nutrition, Remy said. “A quieter environment can also reduce anxiety and physiologic stress. Many patients feel vulnerable in the hospital, and persistent noise can amplify feelings of fear, loss of control, and hypervigilance. In both general wards and ICUs, excessive noise may contribute to agitation, elevated heart rate, and higher perceived pain. A calmer setting can help patients feel safer and more settled, which may reduce the need for sedating medications.”
Another key benefit is reduced risk for delirium and improved cognitive clarity. Older adults, critically ill patients, and those with baseline cognitive impairment are particularly sensitive to environmental overstimulation, he summarized.
“Noise, especially at night, can worsen confusion and disrupt circadian rhythms. Quieter units that preserve nighttime rest and minimize unnecessary stimulation can support orientation and calmer behavior,” Remy said.
