Primary care physicians (PCPs) encounter a familiar paradox: the patient who meticulously tracks carbohydrates, exercises regularly, and takes medications as prescribed, yet still presents with an A1c stubbornly above target. For PCPs managing these “model” patients, success hinges on identifying root causes, adjusting care plans thoughtfully, and sustaining engagement when progress feels elusive.
Why the Numbers Don’t Match the Effort
When adherence appears strong but glycemic control remains poor, the explanation rarely lies in a single factor.
“When patients are doing everything right and still have poor glycemic control, it’s usually because we’re missing a root cause,” said Meena Malhotra, MD, internal medicine and obesity medicine specialist at Heal n Cure Medical Wellness Center in Northbrook, Illinois. “Chronic stress, inflammation, disrupted sleep, or hormonal imbalance can all interfere with glucose metabolism.”
Vanessa Nadia Trespalacios, MD, internal medicine physician with RWJBarnabas Health Medical Group in Livingston, New Jersey, emphasized the unpredictability inherent to diabetes management.
“Stress, sleep, other medications, and even small hormonal changes can impact glucose levels in ways that aren’t always obvious,” she said. “Sometimes, it’s not for lack of effort in making those lifestyle changes — it’s about the body’s response to insulin and metabolism when a patient has diabetes.”
Progressive beta-cell decline, worsening insulin resistance, and comorbidities such as chronic kidney disease, obstructive sleep apnea, or subclinical infection can all undermine glycemic targets. Medication-related issues — variable absorption, suboptimal dosing, or timing errors — may also play a role, as can unrecognized drug interactions.
Psychosocial and contextual factors frequently contribute. Food insecurity, emotional stress, diabetes distress, and depression can interfere with self-management, even when patients are committed.
Sadie Elisseou, MD, internal medicine physician with the US Department of Veterans Affairs Healthcare System and faculty member at Harvard Medical School and Boston University School of Medicine, Boston, noted that lifestyle adherence is rarely static.
“My patients often tell me they were doing great, but then they went on a cruise and drank more alcohol, or they sprained their ankle and couldn’t go to the gym for a few weeks,” she said.
Health literacy gaps also matter. Elisseou recalled a patient whose diabetes wasn’t at goal.
“He told me he didn’t eat any sugar. I asked him to walk me through a typical day — what he ate and drank in a 24-hour period. His daily breakfast was three donuts. In his view, they didn’t have sugar because there was no powdered sugar or icing on them,” she said.
Framing the Conversation
When motivated patients don’t see improvement, how clinicians frame the discussion can determine whether the patient remains engaged or becomes demoralized.
Trespalacios approaches these conversations with explicit reassurance.

“I try to reiterate to them that this is a problem we’re solving together, not a setback,” she said. “I let them know that their effort hasn’t been wasted, and their consistency gives us valuable information about what is not working for them.”
Malhotra uses reframing to shift the emotional tone.
“I’ll say, ‘Your body is communicating something; let’s listen more closely,’” she said. “That shift helps turn frustration into curiosity and keeps them motivated to keep going.”
Avoiding moralistic language is essential. Rather than implying failure, clinicians can focus on partnership and shared problem-solving. Reframing success beyond A1c — acknowledging improvements in energy, fewer hypoglycemic episodes, or sustained lifestyle changes — helps patients see value in their efforts even when lab results lag.
Elisseou emphasized that clinicians have a motivational role that extends beyond prescribing.
“Just because the numbers aren’t at goal doesn’t mean that the patient’s efforts are wasted,” she said. “Their efforts have numerous effects beyond a fasting glucose number, including improvements in sleep, mood, cognition, mobility.” She added: “Clinicians aren’t just there to prescribe drugs — we have a crucial role in motivating and encouraging our patients. Celebrate wins and continue to partner with the patient toward their goals.”
Setting adaptive, short-term goals can also help. Rather than fixating on an A1c target that feels out of reach, identifying incremental wins — such as reducing postprandial spikes or improving medication timing — can sustain momentum.
Knowing When to Adjust
Deciding when to modify the care plan vs allowing more time requires clinical judgment balanced with patient preference. Trespalacios typically allows several weeks to a few months for medications or lifestyle changes to take effect.
“But if trends show persistent highs or lows, or if the patient feels frustrated, it’s time to revisit the plan,” she said. “Sometimes that means adjusting a dose or adding a new medication, and other times it’s about simplifying the routine to make it more sustainable.”

Elisseou framed the decision as a balance between pharmacology and patient preference.
“This is a balance between understanding the pharmacology of how long it takes medications to work and understanding the patient’s wishes,” she said. “Some may prefer to debrief via phone in 2 weeks, and others may wish to see you at their 6-month follow-up visit.”
When glucose levels remain stagnant despite consistency, deeper investigation may be warranted. Malhotra looks beyond glycemic metrics.
“Sometimes that means adjusting medication or nutrition timing, but other times we need to dig deeper and look at cortisol patterns, thyroid function, or microbiome health,” she said.
Objective reassessment is critical. Continuous glucose monitor data can reveal patterns invisible to fingerstick testing. Verifying adherence through pharmacy refills, pill counts, or patient-reported logs helps distinguish true adherence from perceived adherence. Reviewing comorbidities — particularly those that have worsened since the last visit — may uncover new contributors.
Common inflection points for escalation include persistent A1c elevation despite 3-6 months of adherence, recurrent hypoglycemia or hyperglycemia despite dose adjustments, or patient-reported burnout. In these cases, endocrinology referral or intensified team-based support may be appropriate.
Strengthening the System
For patients who are adherent yet struggling, team-based care often makes the difference. Elisseou advocates for interdisciplinary collaboration.
“In my experience, working in partnership with a clinical pharmacist, behavioral health psychologist, social worker, and registered dietician have been crucial to assist our patients who are not meeting glycemic targets,” she said.
Trespalacios echoed this approach.
“Managing these issues successfully often takes a team approach,” she said. “I work closely with diabetes educators, nutritionists, and sometimes behavioral health specialists to uncover challenges that aren’t always evident in lab results.”
Education remains foundational. Trespalacios noted that misinformation often undermines well-intentioned efforts.
“The most important tool is education,” she said. “Often, patients are making great efforts and not seeing positive impact due to misinformation. This is why it is imperative they work closely with the nutritionists and diabetic educators.”
