The US FDA has cleared a new “basal insulin dose optimizer” function for the Dexcom G7 15-day continuous glucose monitor (CGM), which is set to launch separately on December 1, 2025.
The “Smart Basal” feature is designed specifically for adults with type 2 diabetes (T2D) using glargine U-100 long-acting insulin. It will be available within the Dexcom G7 15-day app shortly after the December launch.
The feature is initiated with a prescribed basal dose, then uses the CGM data to recommend adjustments with users receiving daily dose recommendations and reminders within the app.
If low glucose occurs, the Smart Basal will recommend reducing the insulin dose by an amount set by the healthcare provider. Customizable low-glucose alerts are also included. “Once Dexcom Smart Basal is available, healthcare providers can seamlessly integrate it into their workstreams by prescribing and configuring the system using Dexcom Clarity,” according to a company statement on November 19, 2025.
The 15.5-day (including a 12-hour grace period) G7 sensor will initially be available through durable medical equipment providers, including Medicare, with “full retail launch in the coming weeks,” the company said in a November 20 statement.
Initially, the new sensor will work with the iLet Bionic Pancreas and Omnipod 5 automated insulin-delivery systems. Dexcom says it’s working with Tandem and expects to extend the launch “to their customers shortly as they finalize integration.”
